Uplift Seat Assist is an aid to independent living recognized by the US Medicare system to benefit people with a medical necessity for mobility equipment. If you qualify, the lift cushion may be covered for you under code E0629.

If you qualify, Uplift Seat Assist may be covered under US Medicare Code E0629 for 80% of the allowable amount in your state. That means you pay only 20% of the original purchase price. To find out if you qualify, answer the questions below.

		Do you have severe arthritis of the hip or knee, or a neuromuscular disease such as Parkinson's, or degenerative joint disease?
		Do you need help getting up from a regular chair?
		Do you have a red, white and blue Medicare card?
		If so, does it say "Medicare Part B" on the card?

If you answered "yes" to ALL FOUR questions, ask your home medical equipment provider for more information on Medicare billing for the Uplift Seat Assist. He can help you with the paper work, saving you a lot of time and trouble.

If you answered "no" to ANY of the four questions, you do not qualify for Medicare coverage. You may want to consider purchasing our affordable UPEASY brand if cost is a major consideration for you.
